The University of Michigan Health Plan Coverage has Ended.

Please submit your claims timely to ensure appropriate processing.  For details about Medicare claim submission, refer to the Medicare Provider portal for details.   Additional information can be found in the November 2024 communication and FAQ.

All portals still reflect our previous name, Physicians Health Plan (PHP). Regardless of the branding, the following links will take UM Health Plan members, providers, agents, and employers to the proper portals.

Member Reference Desk contains downloadable forms and documents for your health plan. Once you enter your group number and subscriber number, you will have access to documents that may include your benefit summary, member handbook, certificates of coverage, advance directives, privacy statements, pharmacy mail order forms, and prescription drug lists.
